Follow Up Vanderbilt Questionnaire

Click “Follow Up Vanderbilt Questionnaire” above to access the FOLLOW UP Vanderbilt questionnaire. The follow up version will be utilized once the client has taken medication consistently for at least 1-2 months. Please print pages 6-7 for parent input, and pages 8-9 for teacher input. Please read the directions at the top of the page before completing the questionnaire.

*Please note, the Vanderbilt Questionnaire is a screening tool designed to document relevant behaviors of children in both home and school settings. It does not provide a diagnosis. The diagnosis will be made by a trained provider, and the scoring will be completed by our office staff.
